Requesting course evaluation data for courses taught in Submit-a-Course.

We can only provide your course evaluation data if: 1) learners completed the evaluations electronically or 2) learners completed paper evaluations, and you entered their data electronically on their behalf. We cannot provide your course evaluation data if you submitted it via an Excel spreadsheet.

• Instructors and Coordinators can submit an MHFA Data Request Form via the Request Assistance Form.

• The Request Assistance Form can be found in the Resources section of MHFA Connect, under the Request Forms category. 

• When completing the form, be sure to select "I need to request MHFA-related data" as the reason for your inquiry in the first drop-down menu.
